Consent Agreement

By agreeing below, I confirm that I/my child consent to the following:

I/my child agree to take part in gymnastics activities.
I/my child acknowledge that we have read and understood the club’s Code of Conduct and Anti-Bullying Policy and agree to follow these policies.

I/my child consent to being photographed or filmed during sessions for coaching and instructional purposes.
I/my child consent to being photographed or filmed during club activities and events, and for these images to be used in promotional materials such as club websites, social media (e.g. Instagram), newsletters, leaflets, presentations, and newspaper articles.
I understand that consent can be withdrawn at any time.
Please note: It may not be possible to remove images already published or used in promotional materials.

I/my child confirm that, to the best of my knowledge, I am/my child is fit and healthy, and I have provided all relevant medical information that the club should be aware of before participating in gymnastics.
I/my child consent to receiving emergency first aid or medical treatment if deemed necessary by a qualified first aider or medical professional. I understand that all reasonable efforts will be made to contact a parent or emergency contact in such situations.

I/my child consent to the club sharing relevant information with British Gymnastics and its subsidiary organisations (Gymnastics Enterprise Ltd and the British Gymnastics Foundation) for the purposes of membership, insurance, and communications about gymnastics-related services and products.
I understand that I can opt out of sharing my personal information with Gymnastics Enterprise Ltd or the British Gymnastics Foundation.

I/my child confirm that all information provided is accurate to the best of my knowledge, and I agree to notify the club of any changes.
